'We would like to thank our patients for their understanding and patience during our change of computer systems. We still have a lot to learn and would appreciate if you bear with us over the next few weeks. We are hoping to resume full service shortly and commence our new on-line patient access for booking appointments and ordering prescriptions by mid November.'

Patient Access

Patient Access has now replaced Patient Services.

Patient Access is faster, easy to use and works perfectly on smartphones and tablets as well as your computer.

Existing on-line service users who wish to register for the extended facility of access to your medical records to view online, export or print any summary of information relating to medications, allergies and adverse reactions must visit reception to complete the necessary documentation and present patient identification to verify you are the patient, e.g. photo ID and proof of address.

Introducing 'Patient Access'

Patient Access is the name of the system being used by Churchdown Surgery to allow you to view, book and cancel appointments, order repeat prescriptions and view your medical records (subject to consent) from the convenience of your own computer at home.

Patient Access replaces our previous online prescriptions system which has been phased out.

Sign-up to 'Patient Access'

To sign up to this service you will need to visit the surgery and request an online registration form.

Once completed with some personal information to allow us to verify your identity a receptionist will print off a registration form with your username and password. This will enable you to access the online appointment booking and prescription request services.

Using 'Patient Access'

Once you have signed up for this service you will be able to:

  • check your booked appointments online
  • pre-book appointments
  • cancel existing appointments
  • order repeat prescriptions
  • access to your medical records (subject to consent)
